JOHNS CREEK -- Emory Johns Creek Hospital has announced the hiring of a discharge advocate responsible for helping former heart failure patients stay healthy.

Anita Rich, RN, a 30-year employee of Emory University Hospital, was recently hired to fill the position and will work closely with cardiologists and internists who treat heart failure to assist patients with "diet, exercise, medications, fluid and salt intake issues," according to a news release.

A long-time resident of Johns Creek, Rich will also "navigate patients and their families through the journey of optimizing their health."

"Our goal is to deliver the highest quality patient- and family-centered care and lower our heart failure readmission rate through improved patient knowledge," said Marilyn Margolis, EJCH's interim chief nursing officer.

"We're pleased to have Anita Rich join our nursing team as the person dedicated to helping heart failure patients stay healthy, and at home, after being discharged from the hospital."
