FOR QUALIFICATIONS

The Gwinnett Village Community Improvement District is soliciting qualification packages from consultants for the Replacement of the Jimmy Carter Boulevard Bridge over I-85.

Packages shall be delivered to the Gwinnett Village CID office no later than 12:00 P.M. local time on Friday July 10, 2009. The Gwinnett Village CID is located at 1412 Oakbrook Drive Suite 181, Norcross, Ga 30093. One unbound original and seven (7) copies should be submitted. Any submittal received after this date and time will not be accepted. Proposals will be publicly opened and only names of submitting firms will be read at 1:00 P.M.

The Request for Qualifications is available at no charge at the Gwinnett Village CID office. You may call 770-449-6515 or email chuck@gwinnettvillage.com requesting a RFQ package. Consultants are encouraged to review or obtain a copy of the recently completed Interchange Modification Report (IMR) for the project. The report may be reviewed at the CID office or a copy may be purchased for $25.

REQUESTS FOR

PROPOSALS

PROJECT: Gary Pirkle Park Playground

OWNER: City of Sugar Hill, Georgia

Sealed Proposals will be received by the City of Sugar Hill, Georgia for a General Contract to design, deliver and install new commercial playground and other various site amenities at Gary Pirkle Park located at the intersection of Austin Garner Rd. and Suwanee Dam Rd.

Sealed Proposals will be received by the City of Sugar Hill at City Hall, 4988 West Broad Street, Sugar Hill, Georgia 30518, no later than 11:00 AM local time, July 29, 2009. Proposals will be publicly opened and read aloud at this location July 29, 2009 at 2:00 PM . The proposals shall be read in such a manner as to avoid disclosure of the contents to competing PROPOSERS during any allowed process of negotiations. Any Proposals received after 11:00 AM on proposal deadline date (7/29/09) will not be considered by OWNER. Proposals will be evaluated by OWNER and the project award is anticipated upon completion of this evaluation. All Proposals become the property of the City of Sugar Hill. Selection of the "BEST PROPOSAL" will be determined by cost, experience in previous like projects, timeline to complete and references. The weight to be placed on each element is as follows:

BASE Proposal:

The scope of work proposed should include the following:

1. There shall be two playgrounds; one for ages 2-5 and one for ages 5-12. Both playgrounds will be covered entirely by a steel shelter (pavilion).

2. Playgrounds must be placed within a 78' diameter with no decks above 8 feet with the highest point of 5-12 playground located at the center of the 78' diameter in order for it to fit properly under the shelter.

3. Playground shall meet all current standards and guidelines of the CPSC, ASTM, and the ADA

4. Playground installer must be certified by the NPSI.

5. There shall be two additional separate areas outside of the shelter that shall include two bay arch swings with slash proof seats and a two bay arch swing with bucket seats.

6. Playground colors must be earth tone.

7. Play surface under shelter area only shall be bonded rubber with a thickness that meets the requirements according to the fall height the proposed playground. Play surface under and around swing bays shall be engineered wood fiber held in by plastic playground borders.

8. Explanation of any deviations to the specifications as outlined in this RFP.

9. Provide a top view site plan for the entire proposed playground area including ground protective area dimensions and 3-D color renderings of the proposed playground.

10. Provide catalogs and literature for all the manufactures products you plan to use in your proposal.

11. Total Cost not to exceed $160,000.

12. Warranties

13. Proper business insurances must be carried.

14. Timeline to complete project.

15. Business qualifications including all certifications, professional and trade memberships.

16. References with phone numbers.

Performance and Payment Bonds are required and to be issued by a Surety listed on the Treasury as licensed in the State of Georgia, or a cashier's check in the amount of ninety-five (95%) of your total proposal. This will be discussed with the contractor chosen to complete this job.

A mandatory Pre-Proposal Conference is scheduled for 10:00 AM local time, Friday, July 17, 2009 at Sugar Hill City Hall at the above address.

Limitations:

All debris resulting from your work should be placed in dumpsters on site and provided by contractor. The owner shall be responsible for marking all utilities not covered by the Utilities Protection Agency.

PROPOSER shall provide one (1) original and three (3) copies of the Proposal . Each proposal must be submitted in a sealed envelope, addressed to OWNER. Each sealed envelope containing a Proposal must be plainly marked on the outside as, "Proposal for Gary Pirkle Park Playground" . If Proposal is forwarded by mail, the sealed envelope containing the Proposal must be enclosed in another envelope to the attention of OWNER at the address previously given.

Any and all Proposals received without the aforementioned criteria enclosed will be returned to PROPOSER.

Contract will be awarded on a stipulated sum to the PROPOSER providing the Best Proposal as determined by the City after consideration of the factors set forth above and upon the completion of any discussion and negotiation process. The City will consider the base proposal plus any alternatives deemed acceptable. The City hereby notifies any and all PROPOSERS that the City will allow discussion, negotiation and revision of proposals submitted prior to award in order to obtain the best and final offers of each PROPOSER. The City shall allow all companies submitting proposals reasonably susceptible of being selected to participate in discussions, negotiations and revisions of proposals. The City shall not disclose the contents of any proposal to any competing PROPOSER and shall publicly open the Proposals in a manner that the contents of the Proposals are not disclosed to other PROPOSERS.

OWNER reserves the rights to waive any informalities or to reject any or all Proposals, to evaluate Proposals, and to accept any Proposal which in its opinion may be in the best interest of OWNER.

No PROPOSER may withdraw his proposal within 90 days of the proposal submission date.

The Gwinnett County Board of Commissioners is soliciting competitive sealed bids from qualified contractors for the Ronald Reagan Park Stream Restoration Project . Sealed bids will be received by the Gwinnett County Purchasing Office, 75 Langley Drive, Lawrenceville, Georgia 30045 until 2:50 P.M. local time on Thursday, August 6, 2009 and then publicly opened and read aloud at 3:00 P.M. Any bid received after 2:50 P.M. will not be accepted. Bid envelope must be marked on the outside with Bid Number, name of Bidder, and date and time of opening. Contractors providing the utility work must have a current, valid Utility Contractors License.

Work required under this contract includes furnishing materials, labor, equipment, etc. for the projects described below:

The goal of this project is to protect and improve water quality, habitat, biology, and the overall condition and health of Bankston Creek in Gwinnett County, Georgia. The Ronald Reagan Park Stream Restoration project includes the restoration of approximately 500 linear feet of Bankston Creek, the restoration of approximately 800 linear feet of an unnamed tributary to Bankston Creek and the construction of three small best management practices (bioretention BMPs) for stormwater discharging into Bankston Creek and the tributary. The project includes: clearing and grubbing; tree removal; grading for the stream segments and the BMPs; the installation of fieldstone cross vanes, fieldstone vanes and fieldstone retaining walls; the installation of a fieldstone lined step pool outfall channel, the installation of one tributary step pool feature with 14 structures; the installation of a rock lined inflow channel and forebay; the installation of a soil reinforcing mat and revegetation of the stream channels, adjacent riparian areas, BMPs and other disturbed area of the project site and the installation of 12-inch HDPE pipe to connect the BMPs to the stream. The project also includes maintenance of traffic, bypass pumping and sediment and erosion control as required for project construction.

The successful bidder will be required to furnish a contract Performance Bond and Payment Bond, each in the sum of 100% of the total amount bid and provide insurance coverage as required by the contract documents.

Bonding Company must be licensed to do business in Georgia, licensed to do business by the Georgia Secretary of State, authorized to do business in Georgia by the Georgia Insurance Department, listed in the Department of the Treasury's Publication of Companies Holding Certificates of Authority as Acceptable Surety on Federal Bonds and as Acceptable Reinsuring Companies and have an A.M. Best rating of A-VI or higher.

All bonds must be submitted on forms provided by Gwinnett County and agencies providing bonds and insurance must provide proof that they meet the criteria outlined in the bid and contract documents.

Retainage Information: Bidders are hereby notified that when awarded, the contract for this work will comply with the requirements of OCGA #13-10-81 which requires that interest be paid on any retainages. Simple interest will be calculated, using the monthly yield from the Local Government Investment Pool, on the (retainage) interest due. Such amount will be paid to the contractor upon project completion.

Bids in the case of Corporations not chartered in Georgia, must be accompanied by proper certification stating that said Corporation is authorized to do business in the State of Georgia.

No Bidder may withdraw his Bid within sixty (60) days after the actual date of the opening thereof.

Bidder agrees to complete the Contract awarded within the "allowable calendar days for completion" from the date of the "Notice to Proceed" and he further agrees that the Owner may retain from the monies which may become due the amount of $1,000.00 per day for each and every day that the final completion of the work may be delayed.

The successful bidder will be required to provide insurance coverage as required by the contract documents. Acceptable Reinsuring Companies and have an A.M. Best rating of A-VI or higher and insurance must provide proof that they meet the criteria outlined in the bid and contract documents.

Award will be made to the contractor submitting the lowest responsive and responsible bid. Gwinnett County reserves the right to reject any or all bids to waive technicalities, and to make an award deemed in its best interest. Bids may be split or awarded in entirety. Gwinnett County reserves the option to negotiate terms, conditions and pricing with the lowest responsive, responsible bidder(s) at its discretion.

This project is expected to be funded by the Clean Water State Revolving Loan Fund through the Georgia Environmental Facilities Authority (GEFA) and, as stated in the bid documents, shall comply with applicable requirements of GEFA and Georgia Environmental Protection Division.
